Online Switch Kit to Lehigh Valley Federal Credit Union

Welcome and thank you for making the switch to Lehigh Valley FCU. We are excited to have you become “A part of it” and know that you will enjoy the experience.

When you come into our office to open your account please bring in a photo ID, $15.00 membership deposit and change of address card if applicable. Print out and bring along the forms below that pertain to your financial needs. We will complete the NEW account information sections and mail them for you.

1. Print and sign the Change of Direct Deposit Authorization form to begin receiving
your pay at Lehigh Valley Federal Credit Union.

2. Print and sign the Change of Preauthorized Automatic Withdrawal form to switch the
automatic withdrawals from your current financial institution to your new account
with Lehigh Valley Federal Credit Union.

3. Print and sign the Request to Close Account form.
